Login page not opening in R12.1.3 with error "Unable to generate forwarding URL. Exception: oracle.a

Hi,

We have upgraded from 11.5.10.2 to R12.1.3 two months ago. All of a sudden, the login page is not opening up but showing error 'Unable to generate forwarding URL. Exception: oracle.apps.fnd.common.AppsException: java.lang.NullPointerException ".

the application log file shows below error:-
