Associating Accounts with CO4

I am having trouble adding multiple accounts to C04.  I have set up CO4 and added Account picklist to the layout and added Account to the related information.  I am able to select 1 account using the picklist on the page layout (which I can see in the Account detail page under the C04 Related info section), but it does not display below in the Account Related info.  Also, I only have a New button on the Account, not Add (which would allow me to add existing accounts).  I have checked the access profiles and Role management and have either
