BI Publisher report not picking up RTF template

I am trying to make this report work and it is not picking up the RTF template.

Even though the RTF template is uploaded, the concurrent program is finishing with a warning.

The log says below.

Post-processing of request 609822 failed with error message:

One or more post-processing actions failed. Consult the OPP service log for details

I think there may be something wrong in the XML data template.

Please find both XML and RTF attached.
