How to use price breaks with a top limit and additional charge per unit?

My client has a towing service that charges different amounts depending on the mileage that you get towed with a max of 100 miles and then after that it's an additional $3.00 per mile.  How do I setup on a price list? I looked a block pricing but not sure what settings to use. Any help would be greatly appreciated!
