Open ui - Attachment

Hi,

We are on 8.1.1.11.3 Open UI  version. I am facing this issue with attachments across entities. For example to create an opportunity attachment; I navigate to Opportunity -> Attachment view (vanilla). I click on New record; a small applet opens (open ui applet). Then i click on browse and pick the file to be attached. The moment I select ok; I can an error message 'Attachment Name' is required and then the attachment gets saved. This is happening across the application where we can do attachment. I tested this in firefox as well as Chrome and issue is happening for
