Possible environment issue for Oracle 11g R2 Linux 86x64

What:  Oracle 11.2.0.3.0, RHEL 5

I had a database "die" the other day, well, actually you couldn't log into the Oracle server itself until it was rebooted.  The errors all "felt" like an undersized UNDOTBS1 (ORA-00445, and similar errors indicating an out-of-date snapshot, and "process xxx did not start after 120 seconds).  So, I did the "little things":  I added a datafeil to UNDOTBS1 (needless, as it turns out, because OEM history shows that UNDOTBS (autoextend, unlimited size with one file) never filled up.  The Root Cause is baffling.  No space issues on the server itself (even in tempfs).  It's pretty obvious that processes weren't
