WEB ADI Disappears from designated form

in Reporting - EBS (MOSC) Question

Hello,

I am trying to create 3 web ADI integrators for HR. At first i didn't think that my integrators were successfully attaching to my forms. Then after many attempts, it populated. However, the next time I go into the form, the integrator is no longer an option in my LOV. Has anyone experienced this before?

Any assistance is appreciated. You may also want to note I am on 12.1.3 version.
