Duplicate PM Records

Hi

We are on 9.1.We found there are few records in system with duplicate PM Entry P1207.I am wondering how can be two PM entry for the same Tag and Service type.I tried to add it manually but system is giving me error message, both the records at PM status 01.Please see attached information.
