OMS went down without notification.
This afternoon our OEM application went completely down. We have it configured for HA with two primary OMSes and two standby. It is a 12.1.0.2 implementation (we're waiting for 12.1.0.4 for the upgrade). We didn't know there was a problem until the second OMS had an issue today. We have OpenView monitoring the process in the O/S and will alert us if it is not running. The OMS failed at about 10am on April 10 and the other OMS didn't alert us.
