Where to put the EMS vs EM agent

Hello,

I am going to setup my first EM Grid environment and I would like to know where to install the EMS environment with the corresponding DDL.  I have 1 server with 1 production instance and a 2nd server with 2 instances (1 standby in read only mode and 1 QA). I would like to be able to monitor all 3 instances.

If it makes a difference all instances are running the same 11.2.0.4 enterprise edition on the OEL 6.2 operating system.  I want to install EM 12c.
