Software needed for a new two tier APEX installation on Linux

I want to install APEX using a 12cR1 database on Linux on a database tier and OHS and mod_plsql on an a linux web tier.  Which software media includes the correct software for the web tier?  11g Middleware?  I do not see a companion cd download for the 12c database.  Thank you.
