Order is imported without order lines from Internal stand alone application (ADF page)

We have a internal ADF system that is used to take the Order Entry. Data from that system is synced to Oracle OM system using the Oracle API (oe_order_pub).

We have seen that in the last few months, few orders were imported to Oracle OM correctly on the header part but the corresponding all lines for those orders are missing. This is not a regular case and has occurred for few cases.

Can you please share few ideas and pointers what may be the potential cause for these and how to track these. Your inputs are highly appreciated.
