EM 12c Targets shown as down

I am relatively new to the 12c EM, I am seeing a lot of targets which are really up and running as being shown as down. Also some targets are not able to be registered.

Has anyone experienced  similar issues and if so request to point to the resolutions or documents that address this issue.
