Menu export to excel / csv question

Folks,

I'd like to get the menu structure into excel. I have the F9000 that would fit the bill nicely.......but for some strange reason....doesn't show all the form id's for the apps. Does anyone know of a better way of getting the vanilla app name / program id / form id out of the system?
