How can I add a long message tip to the Credit Request form using personalizations?

I would like to add a tip to the Credit Request form. Personalizations would imply that this is possible. Ref Screen shot. We are using the Comments Column to capture specific information

Tip

Please enter the following information in the comments:

• Vendor contact name (including name, phone number and/or email address

• Return Authorization (RA) #

• Carrier (if applicable)

• Tracking # (if applicable)

• Return/pickup date
