Item not found on pricelist error due to qp_qualifiers active flag

Experts,

We have pricelist qualifer based on customer name. There was name change at party level (organization name from X to Y). Now processing an order for Y gives item not on pricelist error. Name change is reflected in qualifier screen in pricelists to Y as well.

On furthur research on the back-end shows qp_qualifiers with active_flag = N. I am unable to find this flag in the front-end screen.

What process/program sets the active flag to Y? Same record in cloned instance shows ative flag = Y with no change to last update date.
