Duplicate OP orders created from same single Requisition

Hello All,

Wondering if anyone else is experiencing this.  I have a user that created 2 Purchase orders from the same requisition.  The OP orders are less than 30 seconds apart.  If I have 2 sessions open for P43060 and bring up the same OR in each, I was able to duplicate the scenario and advised that her process change.  The user is telling me that is not what she did.  She had one single application open and it somehow duplicated on its own.

I cannot duplicate the issue any other way.  As anyone else had this issue?  I would greatly appreciate any feedback as I cannot find anything in My Oracle Support that says this is a problem.
