Unable to apply patch using Application Management Suite - Error - Can't find onlinedef.txt

OEM12c PS2 with 12.1.0.2 plug in version

Already applied  Patch 18325151 to see if this helps. But no luck sofar.  Is there any way we can enable debugging for these steps. I tried to enable debug information in EMconsole but it is deleting the files soon after it fails. So I'm unable to look at the files it is executing that are causing these issues. Any guidence to troubleshoot would be highly helpful.
