Account Alias Transaction errors in install base

Why does this error keep occuring and how do we fix it. "The source item instance XXXXXXX, in Subinventory XXXXXXXXXX, for Organization XXX in Oracle Install Base does not exist. Please verify that any receipt transactions were successful."
