SPARC T4-4 64bit OVM 3.1.1 LDOM (ILOM 3.2.1.8.a) Solaris 5.10 update 11 failes, errors

Having errors attempting to install 5.10 update 11 onto OVM 3.1.1 with ILOM Firmware  3.2.1.8.a upgrade (firmware 8.5.0.a) applied:

Preparing System for Solaris install

Configuring disk (c0d0)

    - Creating disk label (VTOC)

Error:  could not create Solaris disk label (VTOC)

Error: Could not lable disks

Error: Could not update disks with new configuration

   Mkdir: :/a/var/sadm/launcher": Read-only files System

Could not update disks with new configuration

Fatal Error

Solaris installation program exited.

Then the '{o} ok boot net' text install dumps one out to /a /tmp/root

The T4-4 came with two 300GB drives. Primary LDOM setup on one of 300GB drive and appears to be working fine.
