How to customize the short text in PO attachment form not to modify the terms & conditions in the Sh

HI,

Requirement is not  to modify the short/long text data(Terms & condition mentioned in the short/Long Text),once PO is approved. Can u please tell how to do the form personalization or is there any way to handle,
