MSSQL plug-in - How to create a custom configuration

Hi there!

Does somebody knows how to create a configuration extension for SQL Server?

I tried to use "SQL" or "Files & Commands" folder's options using Monitoring Credentials, Agent Credentials, and different combinations but I couldn't create something that works.
