LTD & STD Benefit Setup examples for E1 9.1?

Can someone provide a LTD and STD benefit setup example, using employee monthly salary X 70% divided by 100 times a rate of 1.915 for LTD? The new N* table methods don't appear to calculate monthly salary based on annual salary divided by 12.

Your help is greatly appreciated!
