Which table holds the supplier address name?

On 12.1.3 I need to find out when and by whom a supplier address was created.  Somehow even though Address Name is required, this address has none, and no sites were created for it either.  What table do I look in to find the Created By and the Creation Date for just the address?  It doesn't seem to be in AP_SUPPLIERS or AP_SUPPLIER_SITES_ALL.
