Unable to create database on GI 11gr2 using ASM

Hi all...

I have installed GI 11.2.0.4.0 on Solaris SPARC 11... that finished good, but when we tried to create the database on ASM, we get some errors as follow:

ORA-19624: operation failed, retry possible

ORA-19870: error while restoring backup piece

/app/oracle/product/11.2.0

/dbhome_1/assistants/dbca/templates/Seed_Database.dfb

ORA-19504: failed to create file "+SYSTEM"

ORA-17502: ksfdcre: 4 Failed to create file +SYSTEM

ORA-15001: diskgroup "SYSTEM" does not exist or is not mounted

ORA-15040: diskgroup is incomplete


Any comments will be appreciated...
