EBS 12.1.3 iProcurement POETA to GL Account Generation Failure but Combo is good

We are having an issue that just started a couple of days ago.  A requisitioner is using a POETA that is erroring out with the Account Gernation Failure and change account invalid when trying to submit for approval.  All the combo codes and project/award setups have been checked and rechecked are correct but still getting errors.  The POETA or POET code combo is not new but newly used.  Frequently used POETAs or POETs are not failing.  Has anyone ran into this issue before?  It's been working great until a couple of days ago.  Our technical are taking a look but
