Install base counters are not created from Order fullfillment automatically

Hi..I have setup some Usage  items which are IB trackable and i also, setup the counter templates and counter groups with their associations. Somehow, the counter instance, is not created automatically when the sales order is fullfilled. It just creates an IB record w/o a counter ID. What could be the issue?
