How to convert from Form Concurrent Requests to OAF Concurrent Requests

I would like to transition Submit and View Requests from the Forms process to using OAF View and Submit Requests for all Oracle EBS applications/modules.  Does anyone have a document or instructions on how to do this?  What function or functions should be changed or added to each responsibility?
