How to remove a forms personalization

Hi, I have made a Forms Personalization and have suncessfully migrated this personalization to QA and then PROD, using the LDT file. Now the user wants to get rid of it. I know how to Disable the personalization in the QA and PROD using the the menu by disabling it.  BUt we don't want to do that, we want to actually remove the code (personalization) that is in place and not disable it.  How can this be done and then migrated to QA/PROD without using the menu pull down from the form? Thanks. Kevin
