Workflow email notification response mail no comments within Note

We are using 12.13. version

Email notification response (when approve or reject) page's Note is missing between the quotations in place of ‘Please enter notes as required’  its showing blank.Where as when click on 'Request More Information' , the email response page comes with note Question: 'Enter your question here'.

Can anybody suggest how can I include comment/note within quote in place of blank.
