How to upgrade an existing OMS single host EM12c R2 to EM12c R4 on 2 hosts

Hi all...

We are currently looking for upgrade our OMS single host EM12c R2 on AIX 6.1 to EM12c R4 on 2 hosts Solaris 11 (OMS and Repository BD separated)... i am looking for related documentation about this path of upgrade..

Current OMS/Agents version 12.1.0.2.0, Repository Database 11.2.0.3 version, all in same server AIX6.1

New environment OMS/Agents version 12.1.0.4.0 on host Solaris 11, another one Solaris 11 to Repository Database 11.2.0.4 version

Any comments will be appreciated,
