Adding UDC to form design

I am currently working on Form Design on E1 9.1. I would like to add an UDC like a Branch Plant so that the user can click on visual assist and be able to view the list of Branch Plants and pick and select the relevant ones. In addition I would also like to add a field that would direct users to pick any one value from a customized UDC like 55/01. Please help.
