Creating Service Request - Repair Number - FRM-40735: ON-INSERT trigger raised unhandled exception

Hi, was wandering if anyone could assist with an error we are getting testing Depot Repair.

When a new service request/repair is saved a service request number appears to be generated, we also recieve the "Depot Repair - FRM-40735: ON-INSERT trigger raised unhandled exception ORA-04062".

No Repair Number is generated and the Service Request number cannot be searched on.
