Issue while saving new Withholding Tax Code

Hi All,

 
We are facing attached issue while Saving New Tax Code. During analysis it's found that the issue is faced while we save the record having the check placed against 'Create Withholding Tax Group' check box. And no issue is faced if said check is not placed. However after saving; system allows to place check against 'Create Withholding Tax Group' check box & re/save the record.
 
Please guide in the light of this reply. Also note that we need too quick response; as our business is suffering.
Error Message: 'FRM-40735: POST-INSERT trigger raised unhandled exception ORA-12899'
