What are the causes of OPS Center failing to "Reset to Factory Default" in the pre-tasks to installi

I am struggling to understand why OPS Center is unable to complete the task to "Reset to Factory Defaults" on a T5-2 that I am trying to provision. Does anyone have any ideas?
