Severe system error in different places

Hello,

I am fighting with one annoing issue for a month and I cannot find any pattern.

This happens in different places:

When Returns are created in SIM or RMS Returns request are handled then I receive "Severe System Error" message when trying to add qty to qty field.

When trying to receive the PO by ASN.

When inventory adjustent is being maken.

Firstly I thought it's something connected to Doc ID 1681946.1 and Bug 16977425, but we already have this bug fixed with a patch. Next thing I did was I checked the tablespaces, as written here:
