What is the URL structure for a Contact weblink that will create a new Lead, carry over Contact fiel

in CRM On Demand - Administration (MOSC) Question

Additionally, I need the Contact and Lead to be linked to one another.

So far, I am able to accomplish everything EXCEPT returning the user to the newly created lead.  Here is my working URL (I omitted all of the additional field mappings beyond the first one):
