12.1.3 - Object Meta-Data not showing any source values

I created a new Object Meta-Data so we can relate Quotes to our SR's and the appropriate form opens as we are getting an error.  This works fine.  But if I try to use the same object on a Task in the Source Document nothing shows in the LOV for the Source Value.

I used an 11i article to create the object meta-data, and fumbled my way through to figure out how to get the Quote to relate to the SR, pull up properly and being able to edit. 

I am sure I related something improperly in the Object Meta Data.   We have a Sales Number Object Meta-Data that we can relate to the SR and use in task as a source document which works fine.  If I fill in all the fields on my new Object Meta-Data, like we have for Sales Number,  things stop working on the relating to the SR.
