R12: Install Base Create Form Issue

Hi,

I'm unable to create an Item Instance using Install Base Create Form (Release 12.1.3), I've provided the all the mandatory fileds like Inventory name, PID, Party Detail, Serail Number etc. And the then hit the "Apply" button.

Result: Received error message: "Error". No error should have occured, or if there really was an error, an appropriate message should have been displayed.

Due to this I'm unable to create an Item Instances. Can anyone let me know what went wrong with my Install Base Create Form?
