How to make a new form accessible in submenu options of the Mainmenu?

Hello all Forms Gurus,

Since I have not received any response to my question which I posted yesterday in OTN forms forum, in desperation, I thought it might be helpful to post the same questions in MOSC and I am hoping to receive the help I need to proceed w/ my project. 

Does anyone know why a new form added to the submenu options of the Mainmenu.mmb is gray out?

I added a new form by copying from an existing form, renaming and modifying it.  Then I added the new form to a submenu in the Mainmenu.mmb or our application.  But, the new form is grayed out under the submenu.  The new form was compiled okay without any errors, and I was also able to create an executable file.  After adding the new form to the security form of the application, but the new form is still grayed out (not accessible). What seems to be the problem here!!!  (This is probably a very easy/simple problem to those forms gurus out
