OPP will not failover when database instance crashes. have to manually kill processes and restart

i am currently doing some testing of my CM setup and am having issues with the OPP.  I have setup PCP for my CM's however, everytime there is a database instance crash the OPP will not restart properly.  It shows Actual 0 Target 3

When I click on Processes it shows three processes as Active.  To get OPP working again I have to manually kill these procs from the command line and restart the OPP manager.

Has anyone else experienced this?  How can I get my OPP to failover without manual intervention?
