How does one include county code in the rate distance query using Milemaker when using City to City

We are into OTM v6.3 and wanted to pass the county qualifier to the Milemaker CITY to CITY distance engine call. I have my County already updated on the location in the county_qualifier field. Does the distance engine call automtically sends the county qualifier when I make a city to city call to milemaker else I need to configure that in my enternal distance engine as parameters. If yes how do we do that, since I don't see county as part of the address components to configure it as a parameter.
