How to create a report as same as inquiry funds display on GL Budget

hi all Guru,

Anyone of you that could give me suggest to make a report from which the attached form.
Essentially, I want to create a report similar to the display form inquiry funds.

the problem is we can not find which tables that are used for the display.

perhaps somebody can give me query for this requirement?
