ARM load stuck at pending status

When trying to load data into ARM (initiated from ARM), staging phase is stuck at pending status.

Health checker / validator does not show any issues (SOA Server is running, SOA Composites are all active).

What could be the problem?

The following may be related.  When we try to set up a rate type in ARM, we got the following message. 

     "Financial Data Quality Management, Enterprise Edition is configured with Flatfile.

     The source system and source system rate type fields cannot be retrieved.  You

     may create a rate type without using Financial Data Quality Management,

     Enterprise Edition by leaving the source system and source system rate type fields
