Action History For Requisition is not getting update

Hi,

We have Issue in Purchase requisition approval Process.

Some time our SPR status is APPROVED but action History Shows it is Reject.

It seems action history is not getting update properly.

Please find attachment.

Please help me to resolve this issue.
