SLM: Need additional fields during first or second page and reference them in additional information

Dear All,

We have a requirement from the client where they want to capture vendor type: Manufacturer,Consultant, etc in the first page of supplier registration where he will choose the vendor type and based on the answer the additional information page will populate the UDA's.

e.g if vendor type is consultant.  questions will be like exp,number of consultants, references etc.

Can any one tell how to achieve the same. We have SLM, Supplier Hub and isupplier licences.
