Inventory not hit cardex

Hello

I have got a very unusual scenario that Sales order got processed till sales update, But didn't hit the cardex. I don't see any system crashes during that shipment confirmation process. I also see that F4211.S002 field updated with 1 confirming inventory relieved and hit cardex, though it didn't the cardex.

Looking to understand what could have caused this.
