Attachments uploaded in Forms do not appear in Self Service

Hello,

We have an activity routing attachment for a PM that was uploaded in Forms. We use the attachment category of Operation Attachments.

When a PM is generated, the attachment does not display on the PM Work Order in Self Service.

How do we get the attachments from Forms to display on the PMs in Self Service?
