Autopromotion has failed due to an exception

We have Agile 9.3.3.  We got this error message "Autopromotion has failed due to an exception ORA-00001: unique constraint (AGILE_CHANGE_HISTORY) violated if more information is required, an error has also been thrown to the system log.  Please contact your System Administrator to review the error from the log file."

Does anyone know what this mean?  What causes this error and is there a remedy for it?
