Looking for possible cause of discrepancy between invoice distribution accounting and subledger jour

On two separate occasions we discovered a discrepancy between invoice accounting and subledger journal entry accounting.  We are unable to determine the root cause and do not have any way to determine if this issue has occurred more than twice.  Any information or insight into possible cause would be greately appreciated.
